Your First Visit

A physical therapy evaluation is required for new patients and sometimes returning patients with new injuries. You should expect your first visit to last for about 1 hour. It is important to wear clothing that allows you to comfortably stretch and exercise. We have rooms at our facility available so you can change into proper clothing when you arrive.

There are several documents that we require before your first visit. Please see the below list for required documents. Due to the time needed to complete paperwork, please arrive 15 minutes early if you have not yet completed your paperwork.

Paperwork for Nerve Conduction Velocity/Test

Private Insurance (ex. blue shield)

  • Photo Identification (driver's license, ID card, etc.)
  • Patient Intake Form (.pdf) 
  • Insurance Cards
  • If required by insurance company
    • Referral from your doctor

Medicare and Medicaid

  • Photo Identification (driver's license, ID card, etc.)
  • Patient Intake Form (.pdf)
  • Insurance Cards
  • Referral from your Doctor

Worker's Compensation (ex. work related injury)

  • Photo Identification (driver's license, ID card, etc.)
  • Patient Intake Form (.pdf)
  • Employer at time of injury
  • Date of Injury
  • If available
    • Claim Number
    • Adjuster's Name and Contact Information

Liability (ex. auto accident or legal claim)

Self Pay

  • Photo Identification (driver's license, ID card, etc.)
  • Patient Intake Form (.pdf)
  • Not required but if available
    • Referral from your doctor

 

Please take note that depending on your injury we may have you complete a questionnaire before your evaluation. This helps our therapists better understand your pain level and functionailty. If you have any questions or concerns feel free to contact us via phone.
